FUN WALK ENTRY INDEMNITY FORM

Rules and Information:

  1. The event is organized by the Church of the Resurrection – Bryanston Catholic Church.
  2. Only Walkers that have paid and registered can take part in the event.
  3. Your entry identification form can be collected at Church of the Resurrection on Saturday, 10 November 2018 at the REGISTRATION DESK between 06h00 and 06h45.
  4. On registration, all participants will be issued with a participant number.
  5. Children need to be accompanied by an adult at all times. Children will receive an information tag, parents are urged to complete the information tag and tag children accordingly.
  6. Both Fun Walk Routes will commence at 07h00 at the Church of the Resurrection Parkade on Saturday 10 November 2018.
  7. Participants are to wear comfortable clothes and shoes.
  8. Participants are to wear white t-shirts for identification purposes.
  9. Water points will be provided along the routes.

 

Indemnity: I agree not to hold Church of the Resurrection Bryanston Catholic Church or its Sponsors or any other person assisting in the organization of the event liable for any injury or illness which I or my minor child may suffer as a result of participating in the event, or for any damage or loss of my property which I may suffer directly or indirectly as a result of participating in the event.  By clicking SEND you accept and digitally sign acceptance of this Indemnity for yourself and any child/ren named on the registration under the age of 18 years old.
